Job scope

  • Conduct outreach efforts to raise awareness about mental health and dementia in the community.
  • Identify and support individuals at risk of mental health conditions through screenings and early interventions.
  • Provide case management, basic emotional support, psychoeducation, and link clients to appropriate healthcare or social services.
  • Work closely with healthcare professionals, social service agencies, and grassroots organizations to ensure holistic care.
  • Maintain proper documentation of client interactions and case progress timely.

Job Description

As a Patient Care Coordinator, you will be responsible for delivering high-quality patient-centric care. Reporting to the Nurse Manager/Nurse Clinician, you will ensure that nursing care services are safe, competent, and individualized. You will uphold nursing standards in documentation, treatment, patient monitoring, and infection control.

You will also allocate equipment as necessary to meet care requirements, communicate with patients and their families/caregivers, coordinate patient care, evaluate patients' conditions, and serve as a liaison between families and physicians.

Additionally, you will facilitate multidisciplinary team meetings, observe organizational and legal requirements regarding storage and control of drugs and medical supplies, maintain proper documentation, and monitor and reassess patient conditions.


Required Skills and Qualifications
  • At least a Diploma in Nursing from a local Polytechnic and registered with the Singapore Nursing Board. Post-registration experience in a hospital or hospice setting is preferred.
  • Knowledge in IT and systems.
  • Passion to serve and provide excellent customer service to various stakeholders.
  • Meticulous and accurate.
