As part of our commitment to holistic development, we are seeking a passionate and detail-oriented Social Work Executive to support our Social Worker in case management.
The Social Work Executive will play a crucial role in supporting the Social Work Team at THK Bloom. This individual will assist in case management, liaising with caregivers, coordinating intake processes, and ensuring smooth documentation and reporting. The role also involves administrative duties necessary for effective service delivery and program coordination.
Key Responsibilities:
- Assist the Social Worker in managing and updating trainees' case files, including documentation, assessments, and progress tracking.
- Support the intake process by coordinating referrals, scheduling assessments, and liaising with caregivers and other stakeholders.
- Maintain accurate records of trainees, including case notes, reports, and necessary paperwork for service provision.
- Serve as the point of contact for caregivers and families, addressing inquiries and concerns while providing necessary guidance on available support services.
- Coordinate home visits alongside the Social Worker as part of the intake assessment process.
- Facilitate administrative tasks related to social work functions, including drafting letters, generating reports, and updating databases.
- Support the planning and execution of caregiver engagement sessions, training workshops, and information briefings.
- Work collaboratively with the multidisciplinary team to ensure seamless service delivery.
- Assist in tracking and reporting key performance indicators related to social work services and trainee progress.
- Any other relevant duties assigned by the Centre Manager or Social Worker.
Qualifications
- Diploma in Social Work, Psychology or a related field.
- 1-2 years of relevant work experience in social services, disability support, or administrative roles in a similar capacity.
- Strong organizational skills with attention to detail and the ability to manage multiple tasks efficiently.
- Good communication and interpersonal skills to engage with caregivers, trainees, and multidisciplinary team members.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint) platforms.
