Job Responsibilities:

1) To act as a local, accessible point of contact in agreed areas (east region):

  • Build individual, family and community capacity and resilience through providing advice, information, connections and practical short-term support to anyone in the local community; and
  • Build long-term relationships with between 40 to 60 individuals along with their families and carers to further their emotional growth.

2) To build strong partnerships with communities, agencies and services (including multi-disciplinary teams) to develop and increase their capacity to meet people's needs and those of their families and caregivers.

Administration and information management (10%)
- Organize and contribute administrative records in line with the policies and for the effective operation of the programme and team

Job Requirements:

  • Degree in any related disciplines, preferably healthcare or social work related.
  • A minimum of 5 years' experience in healthcare, social services, or community services. Case Management experience is a preferred but not essential.
  • Experience in positively engaging with individuals and families from diverse backgrounds and with differing needs and abilities, while maintaining a non-judgmental, person-focused approach.
  • Knowledge of policies across health and social care services for people of all ages
  • Excellent communication and negotiation skills to build and nurture trusting relationships and partnerships with a range of people and organizations at a personal, service and community level
  • Experience of setting goals, managing own work and competing priorities, using initiative and exercising judgment

Roles & Responsibilities

The Social Work / Social Service Associate is responsible for supporting initial triage and screening, following-up with intervention plan as per the prescribed care plan under the guidance of the Supervisor, and conducting the engagement and preventive activities to the clients and/or caregivers in an individual or group based settings. He/She will be working closely with fellow colleagues (Social Worker, Counsellor, Psychologist, Programme Executive etc.)in the planning and implementation of targeted screening and cognitive stimulation activities, as well as to monitor clients' condition and progress.

A. Programme Implementation

i. Provide support to clients and/or caregivers through engaging them in meaningful activities such as cognitive stimulation activities or structured activities e.g. reminiscence, memory, linguistic and other appropriate activities) based on clients' interest.

ii. Support in the planning and organisation of structured preventive activities and/or programmes for the community, those at risk of dementia and mental health conditions and clients.

iii. Support in the monitoring the clients' conditions and progress, and escalate to the Supervisor for support.

B. Administration and Staff Development

i. Provide logistical or operational support for clients and/or caregivers.

ii. Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme.

C. Other Duties

i. Any other duties, as assigned

RESPONSIBILITIES
Be at the heart of resident care! As a Case Management Executive, you will coordinate admissions, manage financial assistance schemes, and support families to ensure smooth and seamless care for our residents.

Manage nursing home referrals and admissions, ensuring smooth resident transitions
Extract, organize, and update referral information in digital systems
Prepare and organize admission documents
Financial & Scheme Coordination
Assist residents and families with financial assistance applications
Ensure timely Household Means Testing updates and generate monthly reports
Collect payments, activate GIRO, and coordinate with Finance
Resident & Family Engagement
Support resident's journey from admission till end of life and act as point of contact for them and their caregivers
Address resident and family enquiries related to admissions, financial schemes, and general care, including explaining contents of official letters, where required.
Support resident-centered events such as Family Day and bedside memorials
Administration & Coordination
Maintain accurate resident records and profiles in digital systems
Track outstanding payments, manage resident financial information, and liaise with banks or HDB when needed
Act as Single Point of Contact for AIC matters and ad-hoc requests
Support multidisciplinary meetings and huddles as required
Assist with Medifund administrative requirements
QUALIFICATIONS
