Responsibilities:

Responsible in providing healthcare and patient care to ensure quality healthcare service standards are met by MDAS Care Services.  The Care Services comprise of Home-Based Care and Centre-Based Care.

1.    Provide personal and hygiene care in activities of daily living

Technical Competencies:

·    Able to prepare simple reports

Job Responsibilities:

• To provide personal and social care to residents according to their needs in accordance to ADH Philosophy of Care.

• To support and encourage the wellbeing of residents, and identify any changes that may be required in the care and support provided. To record and report these changes to the clinical team.

• Monitoring and reporting of vital signs, weight and monthly monitoring of glucose level, and changes in psycho-social status

• Make frequent rounds to check residents' condition
• Turning residents at night to change their sleeping positions.
• Change the residents who are incontinent.

• Help the residents to bed at rest time and bed time.

• To ensure that each resident is empowered to be at the forefront of decision making that affects their daily life within the environment, and encourage them to maintain engagement with relatives, friends and the local community

• Administer prescribed medication in accordance with ADH procedures.
• Assist residents with their prescribed oral medications (under instructions from the Nurse in-charge)
• Assist residents with all activities of daily life (eg. personal hygiene and grooming, feeding, transfers, toileting)
• Maintain hygiene and cleanliness of living environment of residents
• Implement infection prevention and control activities
• Accompanying residents to hospitals and polyclinics.
• Engage resident in day to day activity as per Home schedule
• Attends to emergencies as and when the need arises
• Simple application of ointment/cream and simple dressing for residents.
• Support with the induction and training of new staff
• Attend and contribute of briefings and meetings
• Attend all mandatory training sessions/courses necessary to perform these duties and CPD as required to maintain knowledge and best practice.

Job Specifications

• Minimum GCE 'O' Level or ITE or equivalent

Requirements:

• NITEC in Community Care & Social Services
• NITEC in Nursing
• Certificate in Healthcare Support – Patient Care from HMI Institute of Health Sciences
• WSQ Certificate in Therapy Services, along with relevant experience in nursing care roles such as Patient Care Assistant, Therapy Assistant, or Porter
• Willingness to work rotating shifts, including night shifts, weekends, and public holidays

Responsibilities:

• Assist doctors and registered nurses with basic procedures and treatments under supervision
• Perform basic nursing duties such as wound dressing; document, monitor, and record residents' observations and vital signs, and promptly report any changes to the registered nurse
• Supervise and assist residents with their daily needs, including personal grooming and other daily living activities
• Accompany residents during outings and medical appointments

Work Location

The Welfare Homes provide care for destitute persons, and the work location will be either Bukit Batok, Admiralty Street, or Buangkok Green.

• Acacia Home is a seven-storey facility with a capacity of 250 beds, catering male destitute persons. The workplace is located at Admiralty Street.
• Bukit Batok Home for the Aged is a three-storey facility with a capacity of 195 beds, catering male and female destitute persons. The workplace is located at Bukit Batok.
• Tembusu Home at Pelangi Village is a seven-storey facility with a capacity of 190 beds, catering male destitute persons. The workplace is located at Buangkok Green.
• Thuja Home at Pelangi Village is a four-storey facility with a capacity of 239 beds, catering to female destitute persons. The workplace is located at Buangkok Green.

1.    Assume shift or night duties or be on call whenever necessary.

2.    Assist in wound management incorporating tracking of wound progress; and assist in assessing, documentation and reporting of wound condition to Nursing Officers in managing wound outcome.

3.    Pack, prepare and serve oral medication, topical application and subcutaneous injections.

4.    Assist the Nursing Officers in nursing rounds of Medical Doctor and other Allied Health Professionals (AHP) including Dietitian and Speech Therapist by preparing the required document.

5.    Assist in arranging residents to attend medical appointments, such as scheduling appointments, assigning care staff to accompany residents and arranging transportation.

6.    Prepare and arrange dressing trolleys.

7.    Ensure medical supplies and consumables are sufficient.

8.    Tidy up the clinic including Treatment room and Medication room.

9.    Serving and feeding of residents on Nasogastric / PEG tube.

10.  Supervise and assist care staff in handling toileting needs such as bathing, transferring from bed to wheelchair and commode of residents.

11.  Perform daily bladder washout for residents with Indwelling catheter.

12.  Assist the Nursing Officers to handle any emergency.

13.  Attend to emails, telephone calls and phone messages.

14.  Assist in recording cash donations and/or donations-in-kind in the record book provided.

15.  Assist in performing COVID clearance tests such as ART.

16.  Assist in migration of computer digitalisation when necessary.

17.  Timely documentation in the Care Management System.
