• Clients' Assessments: Monitor, assess, and evaluate clients' health status to implement effective care plans.

Duties & Responsibilities

• Provide casework management for our clients, coordinating with external agencies for financial assistance.
• Develop and implement individual care plans (ICP) for clients, providing therapeutic support as needed.
• Execute ICPs and monitor progress, developing and implementing programs and activities aligned with SASCO values.
• Coordinate with PT and OT departments to assist clients in feeling comfortable and confident.
• Prepare 6th monthly ICP review reports and monthly review records for clients.
• Evaluate existing, new, and prospective clients based on their needs, desires, and limitations.
• Update client particulars and social report/review, conducting casework evaluation and maintaining case notes/documents.
• Provide person-centered counseling for clients and/or families, addressing mental health states and coping difficulties.
• Conduct risk assessments, identifying potential harm to self or others.
• Perform financial assessments and manage funds, preparing periodic reports as required.
• Assist with additional tasks assigned by management.

Qualifications & Skills Required

• A degree in social work, registered social worker.
• At least 4 years' experience in social work.
• Prior experience in a residential setting for the elderly.
• Good knowledge of government financial schemes.
• Ability to work with different groups, including the elderly and stakeholders.
• Excellent written and interpersonal communication skills.
• Ability to generate reports and statistics as required by MSF.

Community Care Associate - Social Service

We are seeking a caring and compassionate Community Care Associate to support our elderly clients in their daily lives. As a Community Care Associate, you will be responsible for providing assistance with basic and instrumental activities of daily living, including personal hygiene, feeding, and transfers.

You will develop and maintain relationships with residents and family members through regular interactions and curate and facilitate recreational programs catered to seniors, including baking, art and craft, exercise, and cognitively stimulating games.
