Clinical Practice Includes:
- Providing general clinical pharmacy services to promote wellness and optimize medication therapy outcomes
- Identifying and defining patient-specific medical problems, recommending and managing patient pharmacotherapy, reviewing, counseling, dispensing, and supplying medications as needed
- Monitoring and adjusting patient drug therapy, resolving and preventing drug-related problems in accordance with departmental policies and procedures
- Promoting adherence to prescribing guidelines and drug use protocols
- Providing professional advice and evidence-based drug information to patients and the healthcare team
- Working with patients, caregivers, and other healthcare providers to ensure safe, cost-effective, timely distribution, and quality use of medication
- Communicating with patients and caregivers to ensure understanding of prescribed drugs and resolve or prevent drug-related problems
- Documenting clinical activities as defined in the competency development framework and department guidelines
- Leading and/or serving on departmental and institutional clinical practice committees and other relevant committees as assigned
Education and Training:
- Developing and implementing educational programs and training for entry-level healthcare professionals and the public
- Planning and contributing to the development of high-quality patient education materials and articles targeted at general professional education
- Leading the development and implementation of pharmacy professional standards, competency frameworks, and assessments for future clinical pharmacists for this ACP telehealth service
- Leading and contributing to continuing professional development strategies
Research Opportunities:
- Initiating and participating in clinical research independently or in collaboration with other healthcare disciplines
- Publishing scientific papers in peer-reviewed journals and presenting findings at key local or overseas scientific meetings
Secondary Duties (10%):
- Leading and contributing to the review and evaluation of services, policies, and processes to identify and implement medication safety and quality improvement initiatives
- Participating in and undertaking responsibility for projects and activities that support the goals of the department and organization at the department or institution level
- Maintaining and updating professional portfolios
- Performing other duties or responsibilities as assigned
Requirements:
- Possess MSc (Clinical Pharmacy) or Pharm D or Board Certification
- Complete a Collaborative Prescribing Course within the last 2 years
- Have a minimum of 5 years of experience in clinical experience (ideally in inpatient care)
- Be registered with the Singapore Pharmacy Council
- Possess extensive and current knowledge of pharmaceuticals, clinical practice guidelines, and protocols
- Possess strong leadership qualities
- Possess good supervisory, negotiation, and managerial skills
- Have excellent clinical pharmacy and evidence-based medicine knowledge and skills
- Have excellent clinical problem-solving, judgment, and decision-making skills
- Have excellent knowledge of drug information research tools, clinical research methodologies, and outcomes evaluation

The role of a Pharmacist in the supply chain is critical for ensuring continuity, quality, and cost-effectiveness of pharmaceutical supplies.
Key Responsibilities
- Manage expectations and work closely with clinical pharmacists &/or physicians on alternate sources & drugs when usual sources have supply issues.

- Participate in key activities and related operational workgroups, quality improvement/productivity projects, management of critical supplies & stocks, attend management meetings at the supporting site.
- Timely sourcing and processing of purchase requests from users – Purchase requests must be fulfilled on time in order not to affect patient's treatment plan.
- Verify and submit requirements of national contracts taking into considerations past usage and future trends of disease treatment protocols that aligns with MOH directives and international guidelines.
- Achieve cost-savings from purchases (Group/cluster contracts and institution quotations/negotiations)
- Manage controlled drugs, distribute effectively and in accordance to requirements of Misuse of Drug Acts.
- Ensure continuity of critical supplies or alternate products during exceptional periods.
- Minimize total value of goods written-off (Pharmacy Store and all pharmacy sections/substores).
- Material /item code creation and updates. Timely and accurate maintenance of PHI pharmacy systems (SAP & dispensing systems) on cost price, selling price, drug class and drug subsidy status/type.
- Handle audit queries in all related areas, stakeholder management at supporting site, and lead and motivate the team at site. Requirements
- Degree in Pharmacy with minimum 5 years' of relevant professional practice experience
