DESIGNATION : Social Worker/Case Worker

RESPONSIBILITIES

The Social Worker is responsible for facilitating change and improving lives for individuals, families, community facing transitional challenges through the provision of casework and counselling.

Job Responsibilities:

• Conduct casework management, including gathering information, conducting psycho-social assessment, identifying common needs and goals for intervention
• Provide case work and counselling to persons with mental health conditions, dementia and their caregivers
• Plan, organise and implement community outreach activities, programmes and group work

RESPONSIBILITIES

The Social Worker works in the field of Social Work. He/She is qualified and trained to provide interventions and programs, perform community development work and uplift the professional practice in Social Work.
