Conduct psychosocial assessments to identify mental or emotional distress and provide crisis intervention and support as needed.
- Comprehensive Care Planning: Assess each patient's unique condition, combining medical diagnoses and personal interviews to develop comprehensive care plans.
Provide necessary support tailored to each patient's health status, personal strengths and weaknesses, and emotional state.
- Resource Evaluation and Follow-Up: Research local support groups and resources, matching them to individual patients' needs and preferences.
Evaluate the effectiveness of services and resources provided to patients and adjust plans accordingly.
Follow up with patients throughout their diagnosis, treatment, and recovery process to assess their mental and emotional well-being and provide ongoing support.

RESPONSIBILITIES
The Social Worker works in the field of Social Work.
