As a Case Manager RN, you will play a crucial role in driving Referral Management and coordinating appropriate sites of service. Your responsibilities will include facilitating Care Transitions and providing Care for High-Risk patients. Collaboration with PCPs, Specialists, and Spectrum Clinical Pharmacists will be essential to ensure coordinated service delivery. This position integrates pharmacy management, care coordination, utilization management, and care transition management.You will have the flexibility to telecommute from anywhere within OH, IN, KY, IL, and MO.
