  • Conduct comprehensive care needs assessments to engage residents in understanding their health status and setting health-promoting goals.
  • Implement lifestyle interventions aimed at modifying and sustaining behaviors that reduce health risks and improve health-related quality of life.
  • Collaborate with the Community Health Team (CHT) to ensure holistic care planning and effective service delivery.
  • Provide guidance and supervision as needed for clients with more complex needs.
  • Foster a supportive environment that encourages residents to take charge of their health and well-being.
Work Environment & Schedule
  • Community-based role with stationing in community posts or conducting home visits.
  • Must be physically fit to travel in the community by public transport.
  • Must be able to work during after-office hours or weekends when rostered.
