• You’ll follow electronic care plans for each resident and administer medication as prescribed, accurately maintaining appropriate records for both.

Assist to Assess Resident's needs on for each shift throughout their stay in Nursing Home.

Conduct rounds to ensure residents are well and safe and report any abnormality for immediate / timely treatment and follow up on progress of residents with NM / DNM / Supervisor / NC/ CI / RNIC / NA.

Job Summary:

• Gather social and financial background of the resident and/or the family prior to the application of financial assistance schemes.

• Update information in the overall referral and enquiries database and information system.

• Ensure timely submission of financial assistance schemes for residents in the home, including Medifund, MFEC (Medical Fee Exemption Card), IDAPE (Interim Disability Assistance Programme for Elderly), PGDAS (Pioneer Generation Disability Assistance Scheme) and others.

Job Description
As a Patient Care Assistant, you will support in providing basic nursing care activities such as:

1. Recording vital signs
2. Assisting patients with general hygiene
3. Changing minor dressings
4. Collecting specimens
5. Performing appropriate nursing care as required

You will also be responsible for the admission, transfer, and discharge of patients, maintaining a safe and clean ward environment, and performing portering and other duties as required.

Requirements
Possess a Certificate in Healthcare or equivalent
Preferably 1 year of experience in a ward setting
Good interpersonal and communication skills
