  This position is responsible for conducting comprehensive assessments to identify client needs and developing personalized intervention plans. The specialist will engage clients in professional and therapeutic conversations to understand their requirements and build trust with the family, particularly children and vulnerable members.

  Key responsibilities include assessment and intervention, case management, program development, post-discharge monitoring, and administrative functions. The specialist will also participate in research studies related to community trends and wellbeing, recommending engagement strategies as appropriate.

  Additional responsibilities include ambassadorship and special assignments assigned by the Director to achieve objectives. 1. Assessment and Intervention

• Conduct comprehensive assessments to identify client needs and develop personalized intervention plans.
• Engage clients in professional and therapeutic conversations to understand their requirements and build trust with the family, particularly children and vulnerable members.
• Document and evaluate case outcomes to ensure effectiveness of interventions.

1. Case Management:

• Coordinate delivery of services with other community agencies and monitor progress.
• Organize and conduct case conferences with external agencies to promote collaboration and improve service outcomes.

1. Program Development:

• Assist in planning and implementing therapeutic group work for specific individuals or target groups.
• Identify and engage clients, especially children and vulnerable family members, for relevant programs and activities.

1. Post-Discharge Monitoring:

• Develop and implement discharge plans for clients, including referrals to community partners for continued support.
• Monitor post-discharge status and outcomes, liaising with agencies on client progress.

1. Administrative Functions:

• Maintain accurate records, update case files, and prepare reports for administrative purposes.

The role | Mō tēnei tūranga mahi :

As a Community Support Worker (CSW), you'll be working autonomously with our Tangata whai ora, supporting them to engage and remain in their chosen community and helping them to work on self-directed goals to regain independence, maintain wellbeing and achieve their fullest potential and quality of life. The CSW is required to participate in support and needs assessment, goal setting, and risk management.

The role also involves collaboratively working with the whānau/family as well as working under the multi-disciplinary framework with the Community Mental Health Centres. You will attend regular clinical review meetings with mental health clinicians and other professionals.

You will:

• Support our tangata whai ora (service users) autonomously
• Help our tangata whai ora to work on self-directed goals to regain independence, maintain wellbeing and achieve their fullest potential and quality of life.
• Work collaboratively with whānau/family and clinicians.

You | Mōu :

We are looking for individuals who share our values and has commitment and passion for supporting people in their communities. You will also:

• Hold a clean full NZ driver’s license.
• Hold the New Zealand Level 4 Certificate in Health and Well-being (or recognised equivalent) or be willing to commence study towards this qualification within 6 months of employment.
• Have excellent communication, interpersonal and advocacy skills.
• Strong technology skills; must be experienced in using Microsoft Office products and familiar with databases
• Have the ability to work individually and collaboratively under a multi-disciplinary setting
• Have a sound knowledge of Te Tiriti o Waitangi and be able to demonstrate bicultural concepts in practice, or willingness to learn
