Medical Social Worker

We are seeking a skilled and compassionate Medical Social Worker to join our team. The successful candidate will work with patients and their families to address psycho-social problems, communicate effectively with healthcare professionals, and liaise with government departments and social services organisations.

The ideal candidate will have a degree in Social Work or equivalent, preferably with Honours degree or Masters, and prior experience in social work or the social agency setting. They will possess excellent counselling and communication skills, and be able to maintain case records and participate in supervision sessions, department meetings, case conferences, and training workshops and seminars.

The selected candidate will also be responsible for carrying out psycho-social research and relevant special projects, visiting patients' homes to assess social conditions and family dynamics, and providing counselling and support. They will represent the department on committees of statutory, voluntary, and community organisation providing social services.

• A degree in Social Work or equivalent, preferably with Honours degree or Masters.
• Prior experience in social work or the social agency setting is preferred.
• Good counselling and communication skills.

Working hours are from 8:30am to 5:45pm (Monday to Friday) with a 45-minute lunch break. The location is at a leading nursing home in Singapore.

The primary responsibility of a Social Care Professional involves delivering personalized care to residents, encompassing the development and implementation of tailored care plans, regular progress monitoring, and ongoing review. This entails fostering a supportive environment that meets the unique needs and requirements of each resident.

Key Responsibilities:
