Role Overview
As part of our multidisciplinary healthcare team, the Registered Nurse will play a key role in delivering program-based patient care in Singapore. The initial focus will be on general health programs, while ensuring high-quality engagement, timely reporting, and close collaboration with General Practitioners (GPs) and other healthcare professionals.
Key Responsibilities
• Participate in Teladoc Health Singapore's health programs, which may include chronic disease
support, wellness initiatives, weight management, or other health-related requests as programs
evolve.
• Work closely with GPs and healthcare professionals to ensure alignment of care plans and treatment goals.
• Conduct follow-ups with patients via phone and email to track health data, collect relevant information, and produce patient reports within an agreed turnaround time (TAT normally within
24 hours).
• Review and discuss management plans with the care team as necessary.
• Provide patient education and guidance to support health and wellness.
• Support other ad hoc tasks related to the health programs.
Requirements
• Registered Nurse with a valid practising certificate from the Singapore Nursing Board (SNB).
• General knowledge and clinical experience in health programs and chronic disease care.
• Strong communication and interpersonal skills to engage with patients and healthcare teams.
• Familiarity with Singapore's healthcare system, referral pathways, and local medical resources.
• Proficient in using digital tools, email communication, and patient management systems.
