Roles & Responsibilities:

• Assist in dispensing medications as prescribed by the doctors under the supervision of the Centre Manager.
• Ensure sufficient drugs are maintained at all times, and exchange or write off drugs with near expiry dates.

The Virtual Care Centre (VCC) provides patient-centred, technology-enabled care to patients in the community. As a key member of the VCC team, you will play a vital role in supporting the medication management needs of patients receiving virtual care.

• You will counsel patients on newly initiated medications by the VCC.
• You will address basic patient queries regarding medication handling, dosing schedules, supply status, and escalate complex cases to the pharmacist.
• You will assist pharmacists in history taking and structured medication reconciliation for requests related to medication top-ups.
• You will identify and document findings before escalation to the pharmacist for clinical review and approval of medication top-up requests.
