We are seeking a dedicated and compassionate Medical Social Worker to provide high-quality care and support to our residents and clients.

• Conduct comprehensive assessments to identify residents' needs and develop tailored plans for care.
• Implement appropriate interventions and case management strategies to ensure optimal resident outcomes.
• Facilitate family conferences and participate in multi-disciplinary team meetings to promote collaboration and effective communication.
• Manage referrals, subvention, and subsidy matters efficiently using experience with Integrated Referral Management System and ILTC portal.
• Employ excellent interpersonal, organisational, and communication skills to build strong relationships with residents, families, and colleagues.
• Maintain accurate and up-to-date documentation of case files and time reviews of financial assistance using administrative and IT skills.

• A minimum degree in Social Work is required.
• Registration with The Social Work Accreditation and Advisory Board (SWAAB) is mandatory.

Medical Social Worker

We are seeking a skilled and compassionate Medical Social Worker to join our team. The successful candidate will work with patients and their families to address psycho-social problems, communicate effectively with healthcare professionals, and liaise with government departments and social services organisations.

The ideal candidate will have a degree in Social Work or equivalent, preferably with Honours degree or Masters, and prior experience in social work or the social agency setting. They will possess excellent counselling and communication skills, and be able to maintain case records and participate in supervision sessions, department meetings, case conferences, and training workshops and seminars.

The selected candidate will also be responsible for carrying out psycho-social research and relevant special projects, visiting patients' homes to assess social conditions and family dynamics, and providing counselling and support. They will represent the department on committees of statutory, voluntary, and community organisation providing social services.

• A degree in Social Work or equivalent, preferably with Honours degree or Masters.
• Prior experience in social work or the social agency setting is preferred.
• Good counselling and communication skills.

This role focuses on client well-being through regular visits or remote engagement, demonstrating empathy and concern while connecting clients with available resources and services.

• Conduct home visits or remote sessions to assess clients' emotional, social, and physical well-being.
• Provide supportive guidance and build rapport with clients and their families.
• Accompany clients to other essential engagements when required.
• Offer personalized advice on accessible resources and programs to meet individual needs.

Working Details:

Working Days: Monday - Friday (44 hours per week)

Shift: Flexible scheduling available (8am to 5pm / 8.30am to 5.30pm / 9am to 6pm)
