ResponsibilitiesIdentify, resolve, and prevent drug-related problems; recommend and monitor patient-specific therapies.
Participate in inpatient rounds and ambulatory care clinics to ensure safe, effective medication use.
Dispense medications and provide counselling to patients on their treatment plans.
Communicate accurate drug information and educate patients, public, and healthcare professionals.
Contribute to clinical or translational research to advance knowledge in specialty area.
Registered with the Singapore Pharmacy Council credited college of Pharmacy.
Registered with the Singapore Pharmacy Council.
Ability to work independently.
Able to work under fast-paced environment.
Good listening and communication skills.

Job Description
The Virtual Care Centre (VCC) provides patient-centred, technology-enabled care to patients in the community. As a key member of the VCC team, you will play a vital role in supporting the medication management needs of patients receiving virtual care.
- You will counsel patients on newly initiated medications by the VCC.
- You will address basic patient queries regarding medication handling, dosing schedules, supply status, and escalate complex cases to the pharmacist.
- You will assist pharmacists in history taking and structured medication reconciliation for requests related to medication top-ups.
- You will identify and document findings before escalation to the pharmacist for clinical review and approval of medication top-up requests.
You will also be responsible for prescription fulfillment and coordination, medication delivery and logistics, and quality and safety initiatives.
To succeed in this role, you should possess:
- Certified Pharmacy Technician Certification (CPTC) or Diploma in Pharmaceutical Science.
- A minimum of 1–2 years of hospital pharmacy experience is preferred.
- Familiarity with pharmacy and hospital IT systems.
- Strong coordination, communication, and interpersonal skills to work in a multidisciplinary care team.
- Highly organized and able to manage multiple responsibilities in a fast-paced, evolving care setting.
