Professional Duties
• Interview and counsel patients and their families to help them alleviate or resolve their medico-social and financial issues by:
a) Screening the social background of all new applicants and identifying
potential cases that need assistance (financial and/or social)
b) Liaising with government and voluntary agencies, churches (where
appropriate) when there are areas of needs
c) Seeking financial support for patients who are unable to pay for nursing
• Joint consultation with medical staff, nurses, other healthcare professionals to discuss plans for total patient management. Participate in patient care conference and family conference.
• Liaise with organisations that provide services (e.g. public assistance, home Rehabilitation care services) to meet the needs of the patients and their families.
• Maintain computerised case records and statistics on patients.

• Prepare social and other related reports for management and other related institutions / agencies on request by the ED/ Senior MSW.
• Undertake to be well informed on government guidelines on healthcare and social services available within community as well as the latest in the field of social work.
Administrative Duties
• Apply for Medifund Assistance and/or other financial assistance for cases that need such help.
• Submit statistics to MOH/AIC
Residents’ Engagement
• Establish rapport with residents and ensure residents settle well in the nursing home
• Organise activities that are of interest to residents and encourage participation
• Participate in the training/orientation of volunteers.
• Work with volunteers in activities planning and implementation
Other Duties
• Initiate ACP awareness with residents and their NOKs.
• Participate in internal and/or external committee/workgroup activities as well as research work as directed by the Senior MSW or the ED.
• Cover the duties of the other staff (MSW, MSWA) in his/her absence.
Job Requirement
• Degree in Social Work or relevant formal qualifications from a recognized university.
• Preferably 2-3 years of relevant experience in a healthcare environment.
• Good Interpersonal and communication skills.
• Energetic, committed and enjoy working as a team.
• Has good experience in social work, including experience in counselling. Some knowledge of health care operations and service processes are essential, with
• Experience in a non-profit healthcare institution being an advantage.

Requirements
- At least 3 years’ experience in patient care in the hospital or clinic setting, preferably with experiences in Paediatric nurse.
- Diploma in Nursing (or equivalent)
- Minimum 2 years’ experience in a leadership role, such as Team Lead or equivalent.
- Proven ability to lead, guide, and motivate a team effectively.
- Strong commitment to patient safety and quality care
- Comfortable to work at least 1 night clinic per week (8:30am to 8:30pm)
- Willing to be cross-deployed when the need arises
- Passionate about going the extra mile to support patients' well-being
- Believes in delivering compassionate, patient-centered service with a positive attitude
