Part of Interdisciplinary Specialist Palliative Care Team
42 Hours 5-Day Work Week (Monday to Friday)
Responsibilities:
Assess and identify the needs of patients and families on admission to any of the services and during the regular review.
Participate in after-hours on-call coverage as per rota.
Perform clinical procedures where appropriate.
Document accurately the assessment of patient and family, goals and plan of care, treatment evaluations and reviews.
Provide orientation for new doctors on attachment.
Participate in educational activities such as Journal Club and tutorials.
Participate in guiding medical students and other visitors on attachment.
Provide briefing and/or debriefing for medical students on attachment.
Facilitate access to appropriate supportive care services together with the interdisciplinary team.
Participate in Quality Improvement and/or Research activities on an ad hoc basis.
Requirements:
Possess full registration medical qualification recognized by the Singapore Medical Council (SMC).
Maintained CME requirements and in good standing with SMC.
At least 2 years of medical practice in the hospital or community, of which one year should be in acute medicine with on-call experience. (Salary commensurate to years of experience)
Work experience in Internal Medicine, Family Medicine, Oncology, Geriatrics or other related specialties would be advantageous.

- Work with management to shape care models for metabolic syndrome and skin conditions.
Primary Care
- Deliver care across in-person and telemedicine (video) consults.
- Comfortable managing common acute and chronic conditions, including:
Hypertension, diabetes, hyperlipidemia, hyperthyroidism.
URTI, UTI, and gastrointestinal infections.

Health Screening
- Recommend appropriate health screening panels for patients.
- Conduct physical examinations for occupational clearance or insurance screening.
- Review and interpret health screening reports; advise on follow-up care.
Clinical Training & Improvement
- Mentor and train clinic staff in basic health screening, triage, and on-site care procedures.
- Participate in continuous improvement of care delivery, operational processes, and patient experience.
- MBBS/MD, fully registered with the Singapore Medical Council (SMC).
- Minimum 2 years of clinical experience in a local primary care or hospital setting.
- Comfortable with IT systems, telemedicine platforms, and digital health tools.
