What You’ll Do
As a Staff Nurse, you’ll bring holistic, person-centred nursing to patients’ homes—ensuring physical comfort, emotional support, and dignified end-of-life care.
Key Responsibilities
Conduct home visits and palliative nursing assessments
Develop and review individualised care plans to manage symptoms effectively
Administer medications and adjust care based on doctor’s directions
Educate and support caregivers in providing end-of-life care
Coordinate referrals to inpatient hospices or community services
Offer emotional and bereavement support to families
Maintain accurate documentation; target minimum of 6 visits/day
Support outreach and fundraising efforts when needed
What You Bring
Registered Nurse with SNB (Diploma/Degree in Nursing)
Palliative Care certification is a plus
Minimum 3 years of nursing experience (community/hospice preferred)
Warm, empathetic, and professional — able to support families with sensitivity
Comfortable with independent home visits
