To provide support to rehabilitation counsellors in case management services for members undergoing psychiatric rehabilitation for recovery and re-integration into the community, and to provide support to members in their recovery.
Principal Accountabilities
1. Provide case management support for residential / day members.
2. Co-ordinate Medifund applications for clients and attend Medifund Meetings.
3. Handle the Household Means Testing (HHMT) and Financial Assistance Declarations (FADF) of members, applying for new applications and renewals.
4. Liaise with external agencies for referrals made by the Centre for
members and accompany them for the purpose made in the
referrals if necessary.
5. Assist in the application of the Home Setup fund and purchase of items for
residential and members.
6. Attend case review meetings, case conference/consultation when required.
7. Maintain proper documentations and records for services provided to members.
8 Assist Counsellors to follow up outstanding fees owed by members.
9. Go for home visitations with counsellors when required.
10. Assist in other duties and functions assigned by the Clinical Manager or
Director.
Special Aptitude & Knowledge
Knowledge of social services and various government agencies such as HDB, CPF etc and MOH means testing protocol and Medifund applications.

+ The Case Manager is responsible for overseeing the end-to-end service aspects throughout the case lifecycle (pre- and post-sale)
+ Pre-sale:

+ Post-sale:
+ The Case Manager will mainly liaise with the Distribution Partners, Policy Services, Product & Pricing teams, Client Services and Claims teams on inforce policy requests, including policy changes, inforce illustrations, claims and other post-sales related transactions. Provides clear information on the necessary requirements to allow for one-touch resolution. Responsible for ongoing and proactive status updates.

**2. Reporting**

**3. Other responsibilities**
+ Perform screening of submitted documents before they are registered via AWD for processing
+ For Retail Business:
+ - Checking for completeness includes submitted documents that are presented at the Advisor Centre
+ Ensure proper filing and archival of all the documents received by the team
+ Handle appeals and garner responses from the New Business, Underwriting & Policy Servicing team; ensure documents received are processed promptly
+ Attend to queries relating to third party services and any other matters relating to policy issuance of servicing
+ Conduct enhanced due diligence checks, AML verifications prior to policy issuance and ensure contracts are issued upon receipt of premiums and required original documents
+ Follow up on outstanding requirements and any form of appeals pertaining to Operations matters
+ Review work process improvement to enhance effectiveness and efficiency
+ Assist in onboarding of new banks

**Required Qualifications:**
