Overview
Assist clients and their caregivers to achieve a seamless transition from the various stages of the care journey (e.g. Hospital to Home) by providing them with customer centric and holistic support services.
Responsibilities
Conducts screening referrals and assessment of potential clients and the home environment.
Develop and perform an “Individualised Care plan” (ICP) involving the caregivers / family members (if possible) for clients.
Monitor clients’ conditions and conduct periodical review of clients’ ICP and conditions.
Conduct financial counselling and provide advice on the application of government subsidies.
Collects payments and issues receipts for service delivered.
Manage hotline (on rotation).
Assists Doctors during the home visits.
Develop “Discharge Plan” for clients who do not require the service.
Conduct health education and talks (To seniors and caregivers in the community)
Assist caregivers in navigating the eldercare and caregiver landscape and refer the caregivers to the appropriate services.
Conduct caregiver training.
Perform other duties as assigned.
Qualifications
Candidate must possess at least a Diploma/Bachelor's Degree in Nursing or equivalent.
Registered with the Singapore Nursing Board.
At least 3 year(s) of working experience in the related field is required for this position.
Experience in Home Care service is an advantage.

Responsibilities
Care Planning:
Develop an “Individualized Care plan” (ICP) involving the caregivers / family members (if possible) for clients and review 6-monthly.
Refer clients to other appropriate community services if required, including discharge from services when not required.
Collaborate with Regional Healthcare Systems and participate in multi-disciplinary discussions.
Supervise the recording of nursing care rendered and other matters pertaining to client.
Preceptorship:
Participate in orientation, guidance and mentoring of new staff and those with less experience to always ensure provision of quality care.
Captain the Health Care Team with competence and professionalism, to meet the rehabilitative, treatment and social needs of clients assigned.
Plan and perform service quality audits and competency assessments to ensure maintenance of clinical quality.
Quality Improvement Project:
Lead discussions, recommend initiatives on Quality Improvement activities.
Clinical Care:
Perform clinical assessments and procedures to detect, manage and meet client’s physical, medical and psychosocial needs whilst respecting his autonomy and independence.
Observe and record treatment rendered by the doctor and nursing staff during treatment and examination of the client.
Administration:
Assist with administrative work (e.g. data entry, minutes taking, development and review of SOP).
Participate in office committees as assigned.
Perform other duties as assigned by the Head of Department.
Requirements
Diploma in Nursing / Bachelor Degree in Nursing or equivalent.
Post Basic Certificate / Advanced Diploma in Nursing in relevant specialty is an advantage.
Must be registered with Singapore Nursing Board.
Minimum 5 years of experience as a Staff Nurse in an acute care hospital.
At least 3 years of experience as a Senior Staff Nurse in an acute care hospital.
Experience in Home Care service is an added advantage.

We are seeking a dedicated Administrative Executive to support our Admin Manager in managing reception, administration, and operational support duties. This role plays a key part in ensuring smooth day‐to‐day operations for our residents, visitors, and staff.
Key Responsibilities
Reception Duties
Attend to incoming calls, visitors, and general enquiries
Manage auto‐gate intercom, public address system, incoming faxes, and mails
Maintain proper filing systems and records
Administrative Duties
Handle admission, discharge, and transport bookings for residents
Arrange financial counselling and update residents’ records
Process billing in the CURA System, issue invoices/receipts, and track payments
Update hospitalisation, home leave, and bed status in systems
Coordinate prescriptions with polyclinics and subsidy applications with families
Prepare social reports, draft letters, and coordinate medical reports
Other Duties
Liaise with Nursing, Physiotherapy, and other departments for services, consumables, reports, and special meal orders
Email discharge notices and handle client feedback
Assist with residents’ and families’ needs (e.g., equipment purchases, therapy sessions)
