We are seeking a skilled and compassionate Nurse to join our team. As a member of our Home Healthcare Team, you will play a vital role in providing high-quality patient care and support to clients in their own homes.
The successful candidate will be responsible for conducting screening referrals and assessments of potential clients and their home environments. They will develop and implement individualized care plans, monitor clients' conditions, and conduct periodical reviews of care plans and client conditions.
In addition, the Nurse will provide financial counseling, assist with payments and receipts, manage a hotline (on rotation), and assist doctors during home visits. They will also develop discharge plans for clients who no longer require our services and conduct health education talks in the community.
Our ideal candidate will possess a Diploma or Bachelor's degree in Nursing and be registered with the Singapore Nursing Board. They will have at least 3 years of working experience in the related field and experience in Home Care is an advantage.
