The successful candidate will be responsible for conducting screening referrals and assessments of potential clients and their home environments. In addition, the Nurse will provide financial counseling, assist with payments and receipts, manage a hotline (on rotation), and assist doctors during home visits. Our ideal candidate will possess a Diploma or Bachelor's degree in Nursing and be registered with the Singapore Nursing Board. They will have at least 3 years of working experience in the related field and experience in Home Care is an advantage.

Minimum 3 years’ experience in healthcare, eldercare, or community-based care, including at least 3 years in a supervisory or operational support role.
Proven ability to coordinate multi-disciplinary teams and support integrated medical, nursing, and therapy services (preferred).
