We are looking for a compassionate Health Care Assistant to support homebound clients under our Home Personal Care (HPC) program. The role involves assisting clients with daily living, simple housekeeping, and medication reminders, particularly for those with dementia.
Responsibilities:
- Assess and monitor clients’ care needs and implement personalised care plans.
- Carry out simple housekeeping duties to support clients’ daily living needs.
