About HPC
We are looking for a compassionate Health Care Assistant to support homebound clients under our Home Personal Care (HPC) program. The role involves assisting clients with daily living, simple housekeeping, and medication reminders, particularly for those with dementia.
Responsibilities:
- Assess and monitor clients’ care needs and implement personalised care plans.
- Carry out simple housekeeping duties to support clients’ daily living needs.
- Perform basic nursing tasks such as simple dressing, wound care, skin checks, and medication packing/reminders.
- Conduct mind-stimulating activities, elder-sitting, respite care, and simple maintenance exercises.
- Provide caregiver training and health education.
- Maintain accurate client records and coordinate with administrators.
Requirements:
- Min. GCE ‘N’ Level / WSQ Higher Certificate or Advanced Certificate in Healthcare Support / ITE approved Healthcare Programme is preferred, but candidates with relevant healthcare or caregiving certification or experience in hospital/community/home care or elderly care may also apply.
- Willingness to visit clients’ homes.
- Passionate to work with the elderly and their caregivers
- Strong organisational skills and attention to detail in documentation and reporting.
- Able to thrive in a fast-paced, dynamic, and multifaceted work setting.
- Good interpersonal and communication skills, with the ability to build rapport with clients and their families, as well with colleagues.
- Proficient in basic IT applications for documentation and scheduling.
- This position is on contract basis
