This position involves managing the one-stop Healthier SG booth and providing assistance to patients for Healthier SG initiatives. The coordinator will also be responsible for assisting with HSG assisted enrolment through small group sharing and one-to-one engagement with patients/residents in outreach settings such as hospitals, community settings, via the mobile app or system application.
The coordinator will explain to patients the benefits of HSG initiatives and the importance of enrolling, making the first visit to review Health Plan with the Primary Care Provider, fulfilling preventive health recommendations, and following up for review annually.
