We are seeking a highly skilled and experienced Senior Social Worker to join our team in Fostering Services. This is a challenging and rewarding role that requires strong leadership and management skills, as well as a deep understanding of social work practices.
The successful candidate will be responsible for managing cases receiving foster care services and providing professional supervision to social workers in the area of case management and group work management.
The ideal candidate will have at least 5 years of relevant experience, with 2 years in a supervisory position. They will also have experience working with children and their families, and hold a Bachelor of Social Work or Masters in Social Work degree.
Responsibilities:- To conduct regular case discussions with social workers and supervise their case management.
- To conduct in-depth assessments and prepare Foster Parent Assessment Reports (FAR), and present findings at the FAR Panel and follow up on decisions and recommendations.
- To conduct all necessary statutory visits, including announced and unannounced home visits, to ensure the placement is able to safeguard and promote the child's safety, stability, and wellbeing.
- To organise and participate in inter-agency efforts in support of the welfare and development of the foster child/CYP.
- To provide a 24-hour hotline for foster parents and ensure that the team follows up on outcomes.
- To supervise and support foster placements to prevent breakdowns in the foster child's existing care arrangements.
- To spearhead case movement through regular team meetings, monthly BT case review sessions, MSF case reviews, COF, CART reviews, etc.
- To identify at-risk situations and develop strategies and intervention plans to address child safety issues and wellbeing.
- To monitor and evaluate the effectiveness of case management and other support programmes and activities extended to foster families.
- To participate in road shows and other community outreach events as necessary.
- To meet regularly with the Head of Fostering to assess case progress and programme outcome/effectiveness.
- To ensure/support/monitor all BT SOPs and participate in audit processes.
- To support/conduct research related to practice.
- At least 5 years of relevant experience, with 2 years in a supervisory position.

Roles & Responsibilities:
Secretariat and Coordination Function
- Lead in working with partner organisations' leaders, including SCC, AAC, HPC+, and HT providers, to align goals, establish roles, and develop strategies and workplan for Toa Payoh sub-region to achieve the deliverables specified for the Integrated Community Care Provider.
- Work with centre managers and ground teams to implement the sub-regional workplan, and monitor and report on the progress of the workplan.
- Organise and facilitate regular meetings and check-ins for centre managers and ground staff to build understanding and work through implementation and other challenges.
- Engage and build networks and partnerships with other community care providers, regional health systems, grassroot organisations and other stakeholders.
- Assess resources and identify opportunities for care integration and coordination collaborations across ICCP partners and with external partners
- Work with ICCP providers to trial joint referral workflow to support care coordination for seniors.
- Work with the AIC's sub-regional Relationship Manager on the reporting and monitoring of indicators.
Administrative Function
- Collect and consolidate the information required for the quarterly progress report
- Ensure that the endorsed report and claim form are submitted to AIC in a timely manner
