Responsibilities

  • Review referrals and conduct biopsychosocial assessments during client enrolment to evaluate strengths, support systems, and needs.
  • Build rapport with clients and families, addressing concerns, promoting positive attitudes, and providing education on care needs and resource access.
  • Provide case management and coordinate with external agencies (e.g. hospitals, polyclinics) for medical and financial assistance.
  • Develop, implement, and monitor Individual Care Plans (ICP) in collaboration with the multidisciplinary team (MDT).
  • Coordinate therapy services with PT and OT departments to support clients' confidence and comfort.
  • Conduct monthly and six-monthly ICP reviews and maintain updated client records and documentation.
  • Evaluate client suitability for programmes and adapt plans based on individual needs and limitations.
  • Provide person-centred counselling for clients and families facing mental health, adjustment, or caregiving challenges.
  • Conduct risk assessments related to self-harm, elder abuse, or family violence.
  • Manage financial assessments, assist with funding applications, and prepare required reports.
  • Support the planning and implementation of structured programmes aligned with organisational values.

Requirements

  • Degree in Social Work, Registered Social Worker
  • Min. 4 years’ social work experience
  • Prior experience working in a senior day care setting is a plus!
  • Excellent written and inter-personal communication skills

RESPONSIBILITIES

About the job

General Duties

  • Work in partnership with the SBP team.

Resident Management

  • Co-lead with Residential Care staff to ensure the resident's safety, health & positive behaviour when they are in the Home and during outings & appointments.
  1. Monitor the residents to ensure that they are present in SBP, house or the activity you are supervising (or their whereabouts are known to you when they are not at the Home) and double check, communicate and give permission for residents to leave areas or activities under your supervision when it is requested or required.
  2. Ensure that they follow and adhere to the structure in SBP.
  3. Monitor the residents to ensure that they are physically safe and do not partake in any risky behaviour or activities which may cause themselves or others physical injury – especially during playtime.
  4. Monitor & report to the Management of SBP of any complaints from residents, family members, volunteers or visitors to the Home.
  5. Monitor & report to the Management of SBP of any signs or suspicions of physical, sexual or emotional abuse or self-harm.
  6. Monitor & manage the behaviour of the residents by encouraging & rewarding good behaviour and by preventing, discouraging or discipline misbehaviour in strict adherence to the rules & discipline procedures of the Home.
  7. Supervise residents and teach rules and provide reminders about safe touch & unsafe touch; and check in with residents to flag any behavioural concerns and issues.

Our team of social workers uncovers and enables the strengths of persons with disabilities and their families through person centred planning and supports them in pursuit of their Good Life - where they are safe, healthy, engaged, connected, independent, included and heard.

We are looking for a Social Worker for our Special Education Schools. In considering if you should send us your CV for consideration, we like you to know that in addition to a degree in Social Work, it is far more important that you enjoy knowing and connecting with people of diverse abilities, uncovering possibilities and harnessing the power of communities.

Key Responsibilities

  • The social worker in our special schools brings to the educational process an understanding of the psychosocial concerns of families and the influences of family, community and cultural differences as they interact with the educational process.
  • The social worker will conduct assessments of client needs, provide counselling, support and crisis intervention and participate in programme and policy development.
  • The social worker works closely with families, social and community agencies and volunteers to strengthen the circles of care around our clients with disabilities.

Key Requirements

  • A Bachelor or Master's Degree in Social Work from a recognized university
  • Accredited Social Worker, or eligible for professional registration with the SASW
  • Experience working in a similar setting or with special needs client will be an advantage
