RESPONSIBILITIES

The Social Worker works in the field of Social Work. He/She is qualified and trained to provide interventions and programs, perform community development work and uplift the professional practice in Social Work. He/She conducts casework and group work as well as implements and evaluates community development activities and programs to address service gaps in the community. He/She provides supervision to his subordinates.

RESPONSIBILITIES

Casework - Engagement & Assessment

• Engage client and significant others (e.g. family, caregivers, etc.) in a therapeutic manner to understand their needs

Casework - Case Review & Documentations

About the job

As a member of Gracehaven Social Services Team, you are expected to provide effective Case Work Services for children, youth and their families. To partner with the different professionals to deliver high quality of care to meet the diverse needs of children and young persons entering the residential service. To ensure that the emotional, social, behavioural, developmental, therapeutic and educational needs of clients are met during the client's stay in the Home. Job Description

1. Case Work Management

• Effective use of therapeutic approaches and other intervention strategies to stabilize and regulate emotions of clients.

1. Community Resources and networking

• Complete all necessary documentation for all clients including admission forms, discharge summaries and other ongoing case records.

• Degree or Graduate Diploma in Social Work from a recognized university
• Accredited as Registered Social Worker with SASW.

Responsibilities

Care Planning & Case Management

• Attend to pre-admission referrals and draw out Individualized Care Plans (ICPs and Needs Assessments for necessary intervention upon admission
• Schedule regular interactions i.e. visitation within families, siblings, or/and extended family members (or/and other caregivers)
• Make regular home visits with families or meetings with external/community resources i.e. schools, SSAs, the healthcare sector in gathering insights and recent developments/updates
• Work with families to equip them with the necessary skills and assistance to be able to manage and provide adequate care for their children (i.e. financial/legal assistance, job training/placement, parenting classes, substance abuse/mental health treatment etc.)
• Collaborate with various community partners in maintaining and reinstating familial ties for reintegration or/and preparation for self-efficacy in preparing for discharge
• Identify community-based support resources and ensure appropriate planning for independent living
• Assist with transitional placement of the children in securing other permanent/alternative care options
• Make referrals and introductions to other social service agencies for relevant services & programmes for both the children and families

Counselling

• Address the development, emotional and mental needs of the children in providing quality care for the children in residence
• Provide counselling to the children and initiate/coordinate support linkages for the children with internal/in-house programmes or/and external agencies including schools, mental health facilities, private practice etc
• Provide counselling to family members in fostering family bonds and restoring relationships

Administration

• Maintain accurate records and prepare social reports (pre-admission, case review/summary, care plans, discharge etc) as required
• Maintain computerized records of admission, discharge and case review of all children
• Ensure case file documents and recordings are kept up to date
• Prepare and submit monthly statistical reports to MSF and other relevant agencies
• Assist in the development of the Casework Management System

Other Duties

General Duties

• Work in partnership with the SBP team.
• Work collaboratively with professionals from respective social work organizations, the MSF, educational institutions, hospitals and any other organizations involved in the care plan of every child.

Case Management

• Provide counselling support to assigned residents by regularly meeting them at least once a month and ensure that these sessions are maintained in written records in a timely manner.

• Work with various stakeholders & community partners (e.g. MSF & FSC) to plan and work towards family reunification/permanency planning.

Resident Management

• Co-lead with Residential Care staff to ensure the resident's safety, health & positive behaviour when they are in the Home and during outings & appointments.

1. Monitor the residents to ensure that they are present in SBP, house or the activity you are supervising (or their whereabouts are known to you when they are not at the Home) and double check, communicate and give permission for residents to leave areas or activities under your supervision when it is requested or required.
2. Ensure that they follow and adhere to the structure in SBP.
3. Monitor the residents to ensure that they are physically safe and do not partake in any risky behaviour or activities which may cause themselves or others physical injury – especially during playtime.
4. Monitor & report to the Management of SBP of any complaints from residents, family members, volunteers or visitors to the Home.
5. Monitor & report to the Management of SBP of any signs or suspicions of physical, sexual or emotional abuse or self-harm.
6. Monitor & manage the behaviour of the residents by encouraging & rewarding good behaviour and by preventing, discouraging or discipline misbehaviour in strict adherence to the rules & discipline procedures of the Home.
7. Supervise residents and teach rules and provide reminders about safe touch & unsafe touch; and check in with residents to flag any behavioural concerns and issues.
