We are looking for a dedicated and technically proficient Registered Nurse to join our clinical team. This role focuses on the direct delivery of essential diabetes screenings and patient education. Your primary focus will be providing Diabetic Retinal Photography (DRP) and Diabetic Foot Screening (DFS) services on our mobile screening buses and at our centres. You will also contribute to community outreach, conducting point-of-care testing and empowering patients to manage their health effectively.
Responsibilities- Clinical Screening and Patient Care
- Perform high-quality Diabetic Retinopathy Photography (DRP) to detect retinopathy (centres and mobile buses).
- Conduct comprehensive Diabetic Foot Screening (DFS), identify early complication signs, and provide preventive patient education.
- Nurse Counselling and Patient Education
- Provide individualized counselling and education tailored to patients with diabetes.
- Develop and implement personalised care plans to support patients in managing their condition.
- Community Outreach and Point-of-Care Testing
- Conduct point-of-care testing (e.g., HbA1c and lipids) during community health screenings.
- Participate in outreach events to raise diabetes awareness within the community.
- Reporting and Operations
- Lead submission of patients’ reports to referring healthcare providers within mandated time frames.
- Ensure strict compliance with PDPA guidelines and organisational data security protocols.
- Patient and Operational Duties
- Assist in scheduling follow-up appointments and referrals as required.
- Set up, troubleshoot, and ensure smooth operation of screening equipment for mobile buses, clinics, and other sites.
- Professional Practice and Development
- Deliver patient care according to established standards of care and practice.
- Stay abreast of current trends in diabetes care and contribute to reviewing and formulating care protocols.
- Must be a Registered Nurse with a valid license from the Singapore Nursing Board (SNB).
- A minimum of 5 years of clinical experience (hospital or community settings), with a background in diabetes care and management.
- Strong clinical reasoning, analytical thinking, and ability to provide nurse counselling and patient education.
- IT proficiency for accurate documentation, data entry, report submission, and troubleshooting digital systems, including DRP equipment.
- Certified competency in Diabetic Foot Screening Assessments and Diabetic Retinopathy Photography.
- Experience leading volunteer nurses during community health screening events.
- Comfortable engaging the public on preventive care and health awareness at outreach events.
