Youth Development Centre will provide youth mental health programmes, runs a drop in centre and provides specialised supervision for access of child protection involved children with their parents. We are growing our youth mental health services and are looking for a passionate and dynamic Social Worker to join us.
Serving mostly youth mental health clients, you will need to (i) provide professional services to assess, plan and execute interventions for clients through the support of volunteers; (ii) reach out to youth and their families; (iii) grow our volunteer pool; (iv) ensure safety of the vulnerable children, their families, volunteers and staff through good risk assessment and management. You will also support in programme development, execution and evaluation, as well as other services in the Children and Youth Group (CYG) as and when required,

MAIN DUTIES & RESPONSIBILITIES:
1. Outreach and Case Management/Supervision
1.1 Participate and contribute to outreach to youth with adverse childhood experiences as they have higher risks for mental health issues
1.2 Provide case management to support cases allocated to you, including assessment, case planning, intervention and review.
1.3 Coordinate and facilitate internal case discussions and networking and case conferences with other agencies.

The KidSTART Practitioner's (KSP) role is to provide targeted assistance to children and families enrolled onto the KidSTART programme via a home visitation model. The KSP will work with parents, caregivers, and relevant systems in the following areas:
- Building child development knowledge and parenting skills

- Supporting parents in enabling their children to meet age-appropriate developmental milestones
- Child safety and well-being
- Key life transitions (Primary School transition support)
This includes but not limited to:

- Process referrals from a variety of sources, including hospitals, community referrals, selfreferrals, government agencies and other SSAs
- Engage families about the programme, seek consent from and determine the eligibility of families for KidSTART programme
- Collate and maintain accurate and complete set of data in SSNet and on internal databases on all referrals and intakes.
B. Needs Identification and Assessment
- Conduct timely and regular screening and assessment using different tools and assessments (training provided) to ensure child's developmental needs are met and support maternal/main carer's well-being
- Develop holistic child-centric case plans to coordinate the required support for the identified children.
C. Case Coordination, Systems Linkages and Mobilisation
- Work closely with community partners/stakeholders to enhance service delivery and promote positive child development outcomes
- Where needed, organise case conferences involving the relevant parties to coordinate support and address barriers in support for the identified children and families
- Tap on a team of multi-disciplinary professionals to support health and developmental needs of identified children
- Coordinate support for the identified children and their families through relevant KidSTART support or other resources.
D. Education & Equipping
- Conduct regular parent engagement sessions to enhance caregivers' knowledge, skills and parenting capacity based on the KidSTART curriculum.
- Upkeep and maintain all required administrative documentations including case notes.
Qualifications
Degree in Social Science / Social Work or equivalent. Those without Social Service qualification will be taken in as Executive (Social Work).
The role will be based out of Woodlands-Sembawang area.
Requirements:
- Experience in working with families, undertake case management, assessment writing
- Agility in adapting to changes and the rigours of a home visitation model
- Willingness to conduct some home visits at night
- Able to converse in basic Mandarin to cater to Mandarin-speaking clients, will be a plus.
