We are seeking an experienced Senior Social Worker to join our team. As a Senior Social Worker, you will be responsible for providing high-quality casework and counseling services to vulnerable families.

• Casework and Counselling: You will identify and assess risks in complex cases, using professional tools and templates to guide staff in case assessment and planning.

You will also represent the centre in its advocacy efforts, advocating for vulnerable families to have access to equal opportunities through collaborative practice with community partners.

• Groupwork and/or Community Work: You will lead client / community assessments, adapt group/community work processes, and integrate best practices into group / community work.

You will monitor supervisees' compliance to professional processes, assess their capabilities, and build training interventions to meet growth areas.

You will also induct new hires in the centre through individual supervision during their probation period.

• Operational Managerial Skills: You will support Lead Social Workers / Lead Social Service Professionals in implementing centre's operational and practice protocols, processes, and policies.

• Minimum Bachelor in Social Work or Post Graduate Diploma in Social Work from an accredited institution / Degree in Counselling, Psychology, Social Work from an accredited institution.
• At least 8 years of relevant experience in social work practice.
• Inclination towards research and planning is an advantage.

RESPONSIBILITIES

The Social Worker works in the field of Social Work. He/She is qualified and trained to provide interventions and programs, perform community development work and uplift the professional practice in Social Work. He/She conducts casework and group work as well as implements and evaluates community development activities and programs to address service gaps in the community. He/She provides supervision to his subordinates.

A team player who is committed and service-oriented, the Social Worker works in institutional settings, communities, social service agencies, and hospitals. He also works in collaboration with other agencies and ministries in the course of his work.

Responsibilities

·    To provide case management/ supportive counselling for seniors and their family caregivers. The social worker is to conduct psycho-social and financial assessments, including para-counselling. Makes appropriate recommendations and to carry out an effective clinical intervention for the clients.

·    To plan, develop and implement programmes/services according to the needs of the seniors/community and family caregivers.

·    To plan and facilitate in caregiver support group/ group work

·    To conduct home and hospital visits

Requirements

·    Must have a Degree or Post Graduate Diploma in Social Work or Masters in Social Work

JOB RESPONSIBILITIES

• Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.

Key Responsibilities:

• Provide direct social work case management, counselling to clients and their families, conducting social and financial assessment.

• Update clients particulars and social report/review. Conduct casework evaluation, maintain case notes/documents.
• Person centred counselling for clients and/or families - counselling clients who suffer from poor mental health states (e.g. depression, anxiety), and coping and adjustment difficulties in their transition.
• Risk assessment - assessing risk of self-harm (e.g. suicide) and to others (e.g. family violence, elder abuse).
• Financial assessment and fund management and prepare periodic reports as required - identifying and referring cases for financial assistance (e.g. generate Means Test Subsidy Change Rate Report from MOH for invoices printing/Changed Subsidy report, IHDC Master
• Any other additional tasks assigned by the Management.
