Secretariat and Coordination Function
- Lead in working with partner organisations' leaders, including SCC, AAC, HPC+, and HT providers, to align goals, establish roles, and develop strategies and workplan for Toa Payoh sub-region to achieve the deliverables specified for the Integrated Community Care Provider (ICCP)
- Work with centre managers and ground teams to implement the sub-regional workplan, and monitor and report on the progress of the workplan.
- Organise and facilitate regular meetings and check-ins for centre managers and ground staff to build understanding and work through implementation and other challenges.
- Engage and build networks and partnerships with other community care providers, regional health systems, grassroot organisations and other stakeholders.
- Assess resources and identify opportunities for care integration and coordination collaborations across ICCP partners and with external partners
- Work with ICCP providers to trial joint referral workflow to support care coordination for seniors.
- Work with the AIC's sub-regional Relationship Manager on the reporting and monitoring of indicators.

- Collect and consolidate the information required for the quarterly progress report
- Ensure that the endorsed report and claim form are submitted to AIC in a timely manner
- Represent the sub-region to attend meetings convened by AIC as required, and report back to the leadership team of the partner organisations
Additional Responsibilities
- Take on additional tasks and responsibilities as required by the management to ensure the continued success of the ICCP function in the sub-region.
QUALIFICATIONS
- Bachelor's degree from an accredited University, preferably in Social Work, Healthcare Management, Gerontology, or related field.
REQUIREMENTS
- At least 5 years of experience in healthcare coordination, community care, or a related field, with a proven track record of managing partnerships
- Experience working with seniors or in senior care services
- Strong understanding of community care delivery, integrated service models, and inter-agency collaboration
- Strong understanding of the senior care landscape and relevant industry regulations
- Strong leadership, communication, and interpersonal skills, with the ability to build relationships and lead cross-functional teams
- Strategic thinking with an ability to align operational priorities with long-term strategic goals and ensure sustainable delivery of services
- A proactive, solution-oriented mindset focused on delivering results.

- Maintains a safe and clean home environment for clients, which may include housekeeping, if the client and/or client's caregiver is unable to do so.
- Attend to basic nursing and personal care needs of clients, e.g., feeding, toileting, etc.
- Ensures the comfort and good personal hygiene of the client, e.g. bed bath / shower bath etc.
- Checks, monitors, and records clients' vital signs such as temperature, pulse, respirations and blood pressure.
- Work with the therapists on implementation of activities and exercises for clients at home.

Requirements
- GCE N level
- Preferably attained training in either of the following courses:
- WSQ Higher Cert in Healthcare Support (Nursing Care)
- ITE Approved Training Centre Healthcare (Home Care) program
- Good communication and interpersonal skills. Ability to converse in major dialects is an advantage.
- Strong organisation abilities.

As a Community Care Doctor, you will provide medical care to SLEC clients, along with advice and support to their caregivers in collaboration with the rest of the care team (e.g. nurses, therapists, social workers, counsellors, centre staff, nursing home staff, etc), both internal and external to SLEC, such that care rendered is effective, coordinated and integrated. The Community Care Doctor also fosters a culture of safety and clinical improvement, contributes to programme development and supports the organisation from a clinical angle.
Responsibilities:
· Provide direct medical care to clients in home and residential-based services in a team setting.
· Provide clinical advice and support to caregivers.
· Coordinate care and referrals to other social service agencies, client's primary/ specialist care physician and other healthcare or community service providers when the need arises.
· Assist in the development of medical practice guidelines, policies and protocols as required for the safe and effective provision of healthcare in SLEC.
· Any other projects that are assigned by the manager.
Job requirements:
· MBBS or MD recognised by the Singapore Medical Council.
· Full registration with the Singapore Medical Council.
· At least 3 years of post housemanship.
· Possess good communication and interpersonal skills.
· Willingness to serve and connect with the elderly and underprivileged.
· Relevant Graduate diplomas in Geriatric Medicine, Palliative Medicine or Mental Health would be advantageous.
· Experience working with elderly or palliative patients would be advantageous.
· Proficient in the use of Microsoft Office applications.
· Willingness to travel within Singapore.

Assist to serve pre-packed medication, assist with eye drop administration and apply prescribed topical medication.
- Monitor health status (vital signs, glucose level and oxygen saturation) and escalate to nursing staff when required.
- Perform CPRE / AED and first aid to seniors and plays an active role in emergency situations.
- Adhere to infection control practices
- Maintain cleanliness and safety of work areas
