Role Summary


As a qualified Pharmacist, you will provide high-quality pharmaceutical care as part of our multidisciplinary team.



• Conduct thorough reviews of transfer medications and contact patients beforehand to ensure medication supply is adequate
• Deliver comprehensive medication counselling on transfer medications through various channels, including in-person and virtual sessions
• Perform meticulous medication reconciliation to guarantee accurate medication lists
• Verify new orders from physicians and identify any potential issues
• Assist with medication supply duties and follow up on preparation status
• Undertake additional tasks assigned by management as required


• Bachelor of Science in Pharmacy from an accredited institution by the relevant regulatory body
• Minimum 2 years of relevant experience
• Fully registered with the relevant professional body
• Excellent communication, interpersonal, and organisational skills
• Ability to work independently and as part of a team
• Availability to work weekdays and/or weekends
