Purpose of the Job

Responsible for caring for clients with various medical conditions in the Senior Care Centre. To perform direct care duties including nursing care, elimination management, attend to clients' hygiene & grooming, monitor vital signs, ensuring clients' safety during care, perform simple nursing procedures under supervision, diet management including assistance with feeding, handles medical emergencies, conduct activities and support clinical administrative duties.

Key responsibility areas

• Perform vital signs monitoring and application of care for abnormal readings
• Perform baseline checks like taking height and weight for clients on ad-hoc on monthly basis
• Ensure safe transfer and ambulation of clients
• Attentive and reports clients' abnormal skin integrity
• Ensure safe restraints of clients on wheelchair under doctor's instruction
• Assist in both group and individual activities for clients
• Assist clients with all activities of daily living and report progress to Centre Manager and therapists
• Elimination management such as toileting and changing diapers
• Assist in safe preparation of food (example cutting, blending, reheating, thickener)
• Feeding of clients and ensure proper supervision of clients during meal times
• Attentive to persons with dementia and other mental disabilities
• Activate clinicians and assist them in emergencies
• Safe disinfection of rooms, furniture, equipment, used items and others
• Washing of utensils used by clients
• Handle blood or other body fluid spillages and safe disposal of waste
• Escorting clients on transport to & from centre/clinics/medical institutions
• Ensure safety of clients in transport and complying to safety measures.
• Maintain PDPA and centre operational matters in confidentiality
• Perform other duties as assigned by the Centre Manager/Management

Required qualifications and experience:

• GCE levels / WSQ Certificate in Healthcare Support (Nursing Care). Minimum Diploma in healthcare or equivalent for overseas qualifications.
• Minimum 1 year or above working experience in a Senior Care Centre, Nursing home or with similar settings.
• Trained in BCLS or CPR or first aid.

Required Competencies:

• Must be able to assist in lifting or transferring of clients,
• Able to converse in simple local languages and dialects.
• Able to handle medical emergencies and take lead in absence of clinicians.
• Reliable team member, possess positive work and good interpersonal skills.

(What the role is)

The CDA is a new government agency that is being established by the Ministry of Health (MOH) to oversee Singapore's communicable diseases preparedness, prevention and control, surveillance, risk assessment and outbreak response.

As a Senior/Public Health Officer of the Immunisation Policy and Strategy (IPS) Division, you will be contributing to the Division's objective of minimising morbidity and mortality of vaccine-preventable diseases of public health importance through shaping national immunisation policies and strategies.

You will be working in a fast-paced and dynamic environment that would require the ability to manage multiple priorities and stakeholders at the same time.

(What you will be working on)

You will ensure the smooth function of the National Immunisation Registry (NIR), including attaining high quality data, monitoring, reporting and analysing immunisation coverage in Singapore in relation to prevention of vaccine-preventable diseases. You will oversee the functions of NIR by working with the relevant stakeholders, including providing supervision and training to the vendor engaged to provide services to support the NIR functions. In addition, you will manage public enquiries and requests on vaccination related issues involving the NIR from members of public and healthcare professionals.

As the key administrator of the NIR system, you will also be involved in establishing and maintaining system linkages for data flow, ensuring proper data governance, as well as maintaining the NIR system to ensure its operational readiness.

As part of the review of the national vaccination programme, you will monitor the NIR data closely, support the conduct of relevant statistical analysis on an ongoing basis to understand disease trends, vaccination coverage and implications. You will also generate relevant reports to inform policy decisions.

Due to the cross-cutting nature of the work, you will have the opportunity to interact with respective MOH and healthcare stakeholders, in the maintenance and improvement of the NIR system.

Purpose of the Job:

The ACP Facilitator is responsible for supporting seniors in Advance Care Planning (ACP). This includes advocating for ACP within the community and assisting in the development, implementation, optimization, and integration of ACP initiatives in line with national ACP guidelines.

Key Responsibilities:

• Facilitate ACP discussions and complete care plans tailored to clients' needs.
• Ensure clients and families have seamless access to ACP documentation in both soft and hard copies.
• Conduct timely reviews and updates of clients' ACP documents in collaboration with clients and their Nominated Healthcare Spokespersons (NHS).
• Provide feedback to address service delivery gaps and improve ACP processes.
• Assist in evaluating ACP processes and IT systems to enhance service delivery and address gaps.
• Gather and submit ACP data and statistical outcomes promptly, ensuring compliance with reporting processes.
• Provide logistical and administrative support for ACP events, meetings, and networking initiatives.
• Raise community awareness of ACP and encourage greater participation in advance care planning.
• Support national ACP education efforts and participate in research and quality improvement initiatives to enhance ACP programs.
• Apply acquired knowledge and skills to continually improve the ACP program at SSHWS.

• Bachelor's degree or diploma from a recognized tertiary institution in Nursing, Psychology, Social Work, Counselling, or other relevant Allied Health disciplines.
