 Overview 
 Reporting to NUH Healthier SG Office, you will need to facilitate and drive Advance Care Planning for targeted group of patients at NUH. The incumbent will also need to raise awareness of Advance Care Planning in NUH/ NUHS. 
 Job Responsibilities 
 You will be responsible for the following: 
 Engage all suitable referrals in Advance Care Planning discussion and facilitate the completion of Advance Care Plans for patients. 
 Ensure effective communication of patients' Advance Care Plans to health care professionals and other stakeholders. 
 Follow up and review outcome of Advance Care Planning, including honouring of care and treatment preferences (where possible). 
 Follow up on concerns raised during Advance Care Planning discussion, by providing feedback to the healthcare teams and other stakeholders. 
 Work in partnership with stakeholders to raise awareness of Advance Care Planning. 
 Conduct and participate in Advance Care Planning advocacy and facilitator training. 
 Any other duties as assigned by the reporting officer. 
 Requirements 
 Degree in Nursing, Psychology, Social Work, Counselling or Allied Health relevant to the job. 
 Must be a critical thinker, self-motivated, able to work as part of a multi-disciplinary team, and have excellent interpersonal and communication skills. 
 Ability to use local languages and dialects. 
 Equipped with basic computer skills in MS Word, Excel, and PowerPoint. 
 Knowledge in statistics and familiarity with relevant community services will be an advantage.

Work with policy makers and other stakeholders, including but not limited to primary and community care service providers, hospitals, administrators and researchers to drive the:
Expansion and implementation of the ACP programme
- Development and implementation of the 2023 National Strategy for Palliative Care including involvement in palliative care related pilot programmes, service models and funding policies that support, management of capability and capacity and enablers in the ecosystem
 Enhancement of the systems infrastructure and enablers to support the sector so as to enable it to achieve its intended outcomes.
Undertake other tasks as necessary to contribute to Division's objectives in sector development, capability building and outreach (e.g. conducting literature research, data analysis, etc.)
Job Requirements
- At least 5 to 8 years of working experience in service planning and development, with at least 1 year in managerial capacity in a leadership role, preferably in the healthcare or social care industry
 - Experience in policy formulation, planning, strategy development, sector development or project management in the public sector would be advantageous
