The ACP Facilitator is responsible for supporting seniors in Advance Care Planning (ACP). This includes advocating for ACP within the community and assisting in the development, implementation, optimization, and integration of ACP initiatives in line with national ACP guidelines.
Key Responsibilities:
- Facilitate ACP discussions and complete care plans tailored to clients' needs.
- Ensure clients and families have seamless access to ACP documentation in both soft and hard copies.
- Conduct timely reviews and updates of clients' ACP documents in collaboration with clients and their Nominated Healthcare Spokespersons (NHS).
- Provide feedback to address service delivery gaps and improve ACP processes.
- Assist in evaluating ACP processes and IT systems to enhance service delivery and address gaps.
- Gather and submit ACP data and statistical outcomes promptly, ensuring compliance with reporting processes.
- Provide logistical and administrative support for ACP events, meetings, and networking initiatives.
- Raise community awareness of ACP and encourage greater participation in advance care planning.
- Support national ACP education efforts and participate in research and quality improvement initiatives to enhance ACP programs.
- Apply acquired knowledge and skills to continually improve the ACP program at SSHWS.
- Perform any other duties as assigned by the Reporting Officer.
Qualifications and Experience:
- Bachelor's degree or diploma from a recognized tertiary institution in Nursing, Psychology, Social Work, Counselling, or other relevant Allied Health disciplines.
- A keen learner, critical thinker, and self-motivated team player who thrives in a multidisciplinary environment.
- Strong interpersonal, communication, and multitasking skills.
- Competence in Microsoft Word, Excel, and PowerPoint.
- Experience in Psychology, Counselling, healthcare, or community services in Singapore, will be an advantage.
